    Paramount Intake Process is made of three easy steps. 

  • To begin counseling, Paramount requests the client to complete an intake packet. The forms can be located by clicking on this link. 
     

  • After the Intake Packet has been submitted, Paramount will contact the client  (or client's guardian) to schedule an appointment for a mental health diagnostic assessment. The mental health diagnostic assessment consists of question about the client's history of mental health, trauma, reasons for the need of therapy, thoughts, feelings, and personal/social environment. 

  • Once the assessment is complete, the client will be assigned to a provider and a follow up appointment will be scheduled to complete an Individual Service Plan which consists of goals to meet and a treatment plan for the client. 

Once the ISP has been completed, the client will be scheduled to begin regular sessions with his/her provider.
